Requirements for Community Service (CS):
Your CS site must be a Not-For-Profit agency, see below for suggestions and examples.
This list is offered to help you get started. You do not have to use any of these sites, they are merely suggestions.
Inform your Coordinator at MHACF if you plan to do community Service out of state.
It is the responsibility of the Client and their parents to find a Community Service site.
Fax or Mail your completed CS Work Hours and Evaluation Form back to your Coordinator at MHACF the day you finish CS.
